Cancellation

 Appointment Cancellation Policy

Missed appointments and last-minute cancellations adversely affect the availability of appointments for other patients in need of care. To improve access to care for all our patients, we have established the following policy:

 

  1. Upon scheduling your appointment, we will obtain a credit card to keep on file to hold your appointment. Your card will not be charged at that time.

  2. We ask that you please provide our office with 24-hour notice in the event that you need to reschedule your appointment. This will allow us the opportunity to provide care to another patient. A voicemail can always be left to avoid a cancellation fee being charged.

  3.  A 'No-Show', 'No-Call' or missed appointment, without proper 24-hour notification, will be assessed as a $25.00 fee.

  4.  This fee is not billable to your insurance.

  5. If you are 15 or more minutes late for your appointment, the appointment may be cancelled and rescheduled.

  6. As a courtesy, we make reminder calls for appointments, one to two days in advance. Please note, if a reminder call or message is not received, the cancellation policy remains in effect.

  7. Repeated missed appointments may result in termination of the physician/patient relationship.

A copy of this policy will be provided to you on the patient portal. *By scheduling your appointment, you acknowledge that you have read, and understand the information provided above. You also authorize Mindful Medical to charge your credit card as agreed upon for purchases and fees associated with missed appointments and cancellations made under 24 hours of appointment time. Further, you understand my information will be saved on file for future transactions on my account.
